Adoption Application

Thank you for your interest in adopting a dog from It’s a Pittie Rescue. To start the process we suggest you first review the Adoption FAQ page, which addresses common questions, and then fill out the Adoption Application below. To be an eligible adopter, you must be over 21 years of age, complete the Adoption Application and screening process, and agree to a home visit by an It’s a Pittie Rescue volunteer. Note: It’s a Pittie adopts to the tri-state area and will consider applicants within 3 hours of the Chicagoland area. Also, applicants cannot adopt a puppy 6 months or younger in a home without another dog.

  1. Have you or anyone in your home ever been convicted of a felony?
  2. Have you or anyone in your home ever been charged or cited for any kind of cruelty or neglect towards animals?
  3. Are you legally able to own a pit bull type/bully breed dog?
  4. Do you rent or own your home?
  5. If you have a written lease, would you be able to provide a copy?
  6. Has your landlord consented to you adopting a dog?
  7. Does everyone in your home approve of you adopting a pit bull/bully breed?
  8. Are all of your animals up to date on vaccinations?
  9. Have you ever abandoned or surrendered an animal to a shelter/rescue, police station, or person?
  10. Do you plan on taking your dog to obedience classes?
  11. Do you plan on taking your dog to dog parks?
  12. When you are gone, will you crate your dog?
  13. Are you prepared to care for your dog for the next 10-15 years?
  14. Please detail how you would deal with these common dog behavior issues.
  15. Are you prepared for the additional responsibility and scrutiny that comes with owning a bully breed dog?
  16. Are you prepared for the expense of a dog, estimated to be $1000 a year?
